Often, the needle or other tool is pushed through the body and out again, so both ends are exposed. The tool can then easily be twisted, producing a severe pain. The piercing tools are removed from the body after the scene ends, and the wounds are allowed to heal.
Play piercing should never be done except by skilled practitioners. Sterility of the tools and skin is essential to avoid infection.
