A stuff gag is very similar to, and works in the same way as, a ball gag. The victim's mouth is stuffed with handkerchiefs, scarves, socks, knickers, tights (clean or dirty) or bandannas rolled into a ball, acting as a stopper. To reduce the chances of choking, the stuff gag is never pushed all the way into the victim's mouth. Instead, a large part of it hangs out of the mouth, allowing the top to pull it out easily when he/she has to. Sometimes, to ensure further that the gag does not accidentally slip deeper into the bottom's mouth, the captor may use a larger gag.

For a female victim, it is a commonplace in stories to use the knickers that she is wearing (a panty gag. If there are two females, they may have to have each other's knickers to add to teir distress.

Unlike aball gag, this sort of gag does not normallymake the victim drool, because the stuffing wil prevent this.

With enough stuffing, the mouth becomes so full that the cheeks bulge. Some people find this attractive.

If the stuffing is an absorbent material such as cotton, it sucks up all the saliva in the mouth, which can be unpleasant for the victim. If this is not the intention, such stuffing should be thoroughly wet before use.

The stuff gag was first seen on television, most notably Chinese period dramas, where it was lambasted by members of the BDSM community. While it works in preventing speech, a person who has been stuff gagged can easily spit it out by pushing it with his/her tongue. However, for this reason the stuff gag is one of the safest gags to use during self bondage, as the person with his/her hands tied can still spit the gag out if he/she feels any kind of discomfort. However, the risks of asphyxiation and choking are still present for someone who is not careful.

OTM gag or tape gag over stuff gag

This combination prevents speech and cannot be removed by pushing with the tongue. There is a greater risk of asphyxiation and choking than with either type alone.
